Rotura tráqueobronquial fatal, durante intubación endotraqueal anestésica, en una unidad de estancias cortas.

RESUMEN

Se describen los hallazgos clínico patológicos de un caso de rotura traqueal durante la anestesia de una intervención quirúrgica programada de colecistectomía laparoscópica en una Unidad de Estancias Cortas de un Hospital Comarcal, a una paciente de 67 años. La intubación se realizó sin dificultad, utilizando un tubo del n° 7, según la versión del equipo quirúrgico. Poco después de la intervenm­ción, la paciente presentó un cuadro de enfisema tóraco-facial severo, acompañado de sensación de ahogo y quemazón de garganta. La fibrobroncoscopia reveló una rotura traqueal de 5 cm de longitud, localizada sobre la pars membranosa de la pared traqueal, desde la carina hasta la primera porción del bronquio derecho. La paciente fue reintervenida a través de una toracotomía derecha. Durante la segunda intervención se realizó una intubación del bronquio principal izquierdo, aislándose el pulmón derecho. Después de la sutura traqueal se comprobó otra rotura en el bronquio principal izquierdo. El equipo quirúrgico manifestó la existencia de una gran friabilidad de los tejidos traqueobronquiales, motivo por el que se producían continuos desgarros durante la sutura. La paciente murió durante la intervención que se prolongó durante casi cinco horas. Se realizó una denuncia familiar ante el Juzgado de Guardia por posible mala praxis.

 

Describimos los hallazgos de la autopsia médico-legal y los resultados de la investigación histopatológica realizada sobre la tráquea y los bronquios principales, al objeto de descartar patología inflamatoria y/o degenerativa, así como posibles anomalías congénitas de la vía aérea. Se discuten los posibles mecanismos etiopatogénicos de las roturas traqueales durante la anestesia.

ABSTRACT

The authors report a fatal case of tracheal rupture in a 67 year-old woman after endotracheal intubation. The woman suffered a laparoscopic cholecystectomy under general anaesthesia. The anaesthetic team reported straightforward intuba­tion with a 7 mm tube; a stylet was not used. At the completion of the operation, shortly after the trachea was extubated (approximately 1 hour after the intervention), the patient showed impressive surgical emphysema involving the upper half of her body, and was unable to open her eyes. Bronchoscopy revealed a 5 cm tear on the membranous pars of the trachea extending to the carina and to the first portion of the main right bronchus. The woman was taken back to the operating theatre and anaesthesia was induced with propofol, fentanyl and oxygen. She was placed in the left lateral position and a right posterolateral thoracotomy was performed, which revealed a superior mediastinal air sac. When this was opened, a massive air leak resulted, necessitating left endobronchial intubation which was accomplished without difficulty, and the right lung was isolated.

 

After the tracheal suture, a tear on the posterior wall of the left main bronchus was noted. The thoracic surgeon said that ¡he tracheobronchial wall was very weak, and tore continuosly during suture. Five hours after the thoracic intervention the patient was pronounced dead. A medico-legal investigation of the death was ordered.

 

We report the clinicopathologic findings, including microscopical studies of the tracheobronchial tree, in order to investigate the existence of diseases affecting the tracheobronchial wall which could explain the allegations of the surgical team (especially the existence of inflammatory, degenerative and congenital anomalies of the airways).
